Praxis MD is a free, public NPI directory. By using the site you agree to the terms below.

What this site is

Praxis MD is an independent presentation layer over the public NPPES dataset published by the Centers for Medicare & Medicaid Services. We are not affiliated with CMS, HHS, or NPPES, and we are not endorsed by the U.S. government. NPPES is the system of record.

Use of the data

The provider data on Praxis MD is publicly released by CMS for legitimate healthcare-administration purposes. We surface it with a B2B audience in mind — credentialers, billers, recruiters, researchers, and healthcare sales.

You may:

• Read individual provider records for legitimate verification purposes.
• Reference Praxis MD pages in your own work with attribution.
• Use our free tools (NPI Validator, search) for individual lookups.

You may not:

• Scrape, harvest, or systematically download data from this site outside of our published API and tools.
• Use the data for spam, harassment, or any activity that would violate CMS's NPPES data dissemination notice.
• Resell or rebrand the data without independently sourcing it from NPPES.
• Misrepresent NPI presence as proof of active licensure or payer enrollment.

Accuracy

Praxis MD reflects what NPPES contains. If a provider's information is wrong on this site, it is also wrong at the source. Providers can correct their record at nppes.cms.hhs.gov; updates flow into Praxis MD on the next refresh cycle.

NPI presence is not a credential. State licensing boards and payers are the authoritative sources for license status and enrollment.

No warranty

The site is provided as-is, without warranty of any kind. We do our best to keep the data fresh and accurate, but we cannot guarantee uptime, completeness, or fitness for any particular purpose.

Limitation of liability

To the maximum extent permitted by law, Praxis MD and its operators are not liable for any indirect, incidental, or consequential damages arising from use of the site or reliance on its data.

Changes

We may update these terms as the site evolves. Material changes will be noted at the top of this page.
