Hospitalists: from 0 to 109,847 in 20 years

When the Hospitalist taxonomy code (208M00000X) was created in 2006, there were a few thousand providers self-identifying with it. Twenty years later it's the fastest-growing physician specialty in NPPES — concentrated in metros with academic medical centers (Houston, Philadelphia, NYC, Boston). Median NPI age in this cohort is just 8 years.

🏥 Practice-acquisition signal

Sanford Health added 67 NPPES locations across the upper Midwest this quarter — visible as new secondary practice addresses on the same Type 2 NPI. Concentrated in Sioux Falls, Fargo, and Bismarck. For PE / M&A watchers, this is the largest single-quarter footprint expansion visible in 2026 NPPES data.

Workforce concentration in five states isn't surprising — those are the population centers. What is surprising is the slope: in 2020 the top-5 captured 41% of new internal medicine enumerations; in 2026 it's 47%. Six points in six years. If this continues, the rural workforce shortage isn't a future problem, it's a present one with a measurable rate of decline.
