Correct a provider record
Praxis MD republishes public NPPES data. We can fix a display or processing error here, but only the provider or an authorized representative can change the underlying NPPES record.
If the information is wrong in NPPES
Update the source record through the official NPPES website. CMS states that providers or their representatives may edit their records there. Praxis MD will pick up accepted changes through the next CMS weekly or monthly file.

If NPPES is correct but this page is wrong
Email hello@getpraxismd.com with:
- The 10-digit NPI and Praxis MD page URL.
- The field that appears incorrectly.
- A link to the corresponding public NPI Registry record.
Do not send patient information, medical records, Social Security numbers, or other sensitive information.
